Family/Individual Services - Children
Service Goal
The goal of Family/Individual Supports for Children is to provide children with disabilities who live in their family home services/supports individualized to their needs and desired outcomes. Families are the primary decision makers in the process of identifying needs and services and play a critical role, along with team members, in the process. This service is also intended to improve the quality of family life by providing temporary relief from the ongoing responsibility of supporting the child with a disability.
Service Descriptions
Children's Community Based
Family/individual supports/services provide the support for children to be involved in community activities. This includes recreation, extra-curricular school events, and other activities sponsored by community groups.
Children's Supportive Home Care
Supportive home care provides direct assistance or training in daily living skills, socialization, recreation, personal adjustment, and supervision.
