Service Goal

The goal of Case Management is to assist persons with disabilities toward maximum independence, integration, productivity, and successful inclusion in the community through assessment, planning, linkage, advocacy, coordination, and monitoring/follow up.

Service Description

Case Management supports/services assist persons and their support networks to identify and coordinate the use of both paid services and natural supports. Supported services should be consistent with the person's capabilities and preferences, as outlined in the person's Person Centered Plan (PCP).
